Requirements:
- Complete FORM -C.
- Submit original Passport & copy of first 2 pages & any endorsements.
- Documentary evidence for amendment in the Passport.
- Fee in the shape of money order or cashier’s check payable to the Consulate General of Pakistan. Personal checks are not acceptable and fees are non refundable.
- Proof of Residence like copy of state ID card or Driving License (The Houston Consulate’s consular jurisdiction is only for Texas, Louisiana, Oklahoma, New Mexico and Arkansas).
- Prepaid self addressed return envelope for applications sent by Fedex, UPS and Certified mail (US
Postal Service is preferable).
Note: Women intending to change their names due to marriage or divorce must apply for a new passport. Please see passport renewal option.
Children are no longer endorsed on parents’ passports. Please see option for new passport for children born in the US.
Extension of limited validity passport
Fee and processing time:
There is no fee for extension of restricted validity passport. Applications received in mail or in person are processed in 7 business days.
Requirements:
- Complete Form -C.
- Submit original Passport & copy of first 2 pages.
- Two passport size photographs.
- Original NICOP/CNIC. Applicants who have not yet applied for NICOP must submit NICOP application with the passport extension application. (Visit NICOP page).
- Proof of Residence like copy of state ID card or Driving License (The Houston Consulate’s consular jurisdiction is only for Texas, Louisiana, Oklahoma, New Mexico and Arkansas).
