Attestation/Legalization
Attestation of Document
• Notarized copy of the document with the original document.
• Notarized copy of Pakistani passport and CNIC/NICOP.
• A copy of State ID or Driver license.
• Proof of Residence like copy of state ID card or Driving License (The Houston Consulate’s consular jurisdiction is only for Texas, Louisiana, Oklahoma, New Mexico and Arkansas).
• Prepaid self addressed return envelope for applications sent by mail (US Postal Service is preferable).
Power of Attorney Attestation
• Notarized copy of the power of attorney with the original power of attorney.
• Notarized copy of Pakistani passport and CNIC/NICOP.
• A copy of State ID or Driver license.

Important Note:- If you are US national or representing a US company, then all documents have to
be first authenticated by the US Department of State (Authentication Office, 518 23rd Street NW SA-1, Washington, D.C. 20520).
