Child Birth Registration
We offer Birth Registration service for child born to Pakistani parents in the States of our jurisdiction in United States of America. We accept the documents in mail or walk-in between 0930am to 1230pm CST (Monday to Friday) except holidays – for holidays or any changes in timings please visit the homepage.
Standard Requirements
Kindly note the following standard requirements for all types of documents:-
- Original valid Pakistani Passport or CNIC/NICOP photocopy of the parents. If one of the parents is a non-Pakistani, his or her Passport & DL must be attached.
- Complete Form S-1 (please click here to download). Kindly complete the form in all respects preferably typed in black and signed in blue;
- Original along with notarized copy of birth certificate issued by the State/Civic authority (Not of hospital).
- Notarized copy of the Marriage Certificate, reflecting both parents names.
- Two recent passport size photographs of front view with light blue background.
- Proof of Residence in Pakistan Consulate Houston’s area of jurisdiction e.g. motorized copy of state ID card or DL (of Alabama, Arkansas, Colorado, Georgia, Florida New Mexico, Oklahoma, Mississippi, Louisiana & Texas).
- A self-addressed, pre-paid return envelope with tracking number {in case the applicant desires the return of the attested document via post}.
- We do not require you to bring witnesses to the Consulate. However, if you so require, please get signatures from witnesses before visiting this Consulate
- Fee of $30.00 is accepted in the form of Debit/Credit card payable to the Pakistan Consulate General Houston. No cash or personal checks are accepted.
