Documents required to be submitted with Passport application
Issuance of new passport in lieu of Expired Passport
• Complete Form-A.
• Notarized copy of CNIC/NICOP.
• Applicant less than 18 years of age should provide notarized copy of Registration Form CRC or CNIC/NICOP.
• Original expired passport and copy of first two pages & any endorsements.
• 4 Photographs.
• Renewal fee $46.00 for ordinary passport & $120.00 for urgent passport.
• Notarized copy of Nikah Namah in case applicant wants to have husband’s name on passport.
• Proof of Residence like copy of state ID card or Driving License (The Houston Consulate’s consular jurisdiction is only for Texas, Louisiana, Oklahoma, New Mexico and Arkansas).
• Prepaid self addressed return envelope for applications sent by Fedex, UPS and Certified mail (US
Postal Service is preferable).
Express Mail
If application is submitted through mail, self addressed pre-paid envelope is mandatory for the return of documents. The Consulate recommends the use of Express Mail for all deliveries. Please use the flat rate envelop. Do not use Metered stamps. The Consulate does not accept any other form of return envelope. Click here for US Postal Services. Consulate will not be responsible for loss of documents in the mail.
Note :
Passport with one year validity will be issued in the absence of CNIC/NICOPP. The validity of the passport would only extended on presentation of CNIC. The name appearing on the new passport would strictly be as per old passport or national ID card, except in the case of marriages on the submission of notarized copy of Nikah Namah.
It is strongly recommended that applications be mailed with tracking number. The Consulate would not be responsible if documents sent without tracking number are lost in the mail.
Issuance of new passport in lieu of Lost Passport
• Complete Form-A.
• Form-D with one notarized photograph.
• Memorandum.
• Report of Lost Passport Form.
• Police report is required.
• Two copies of lost passport or passport number, date & place of issues.
• Original CNIC/NICOP(With two copies).
• 4 photographs.
• Fee $92.00 for ordinary and $240 for urgent passport (First Time).
• Fee $185.00 for ordinary and $480 for urgent passport (Second Time).
• Proof of Residence like copy of state ID card or Driving License (The Houston Consulate’s consular jurisdiction is only for Texas, Louisiana, Oklahoma, New Mexico and Arkansas).
• Prepaid self addressed return envelope for applications sent by Fedex, UPS and Certified mail (US

In case the applicant does not have original CNIC/NICOP, then he/she should submit original old national ID card (with two copies) along with any 1 documents out of following 7 mentioned below:-
• Original Birth Certificate (with two copies)
• Original Citizenship Certificate (with two copies).
• Original Form-B (with two copies).
• Original Domicile (with two copies).
• Original Pakistani Educational Certificates minimum 2 (with two copies)
• Original Expired and previously issued Passport (with two copies)
• In case applicant has traveled on mother’s passport, then mother’s original passport on which the applicant was endorsed. (With two copies).
• In case of non-fulfillment of requirements mentioned above, the passport will not be issued. Such cases would then be referred to concerned authorities in Pakistan for verification and clearance, before issuing the passport, which will take minimum 10-12 weeks.
• A six (6) months restricted validity passport will be issued in lieu of lost one, which will be extended for full 5 years validity on the receipt verification and clearance from concerned authorities in Pakistan.
